Lower Back & Leg Pain

Lower back pain with associated leg discomfort is one of the leading causes of work absence and reduced quality of life across the UK, affecting around 8 in 10 adults at some stage. The condition encompasses a wide range of presentations, from dull, aching lower back stiffness after long periods of sitting, to sharp, burning sciatic pain that radiates down the back of the leg, to aching knee and hip pain linked to spinal degeneration. Common causes include muscle strain from lifting or overexertion, sciatic nerve compression, lumbar spinal stenosis, osteoarthritis of the spine or hip joints, and general age-related wear and tear. Chronic lower back and leg pain often becomes a self-reinforcing cycle: pain reduces activity, which weakens supporting muscles, which in turn worsens pain and mobility.
yaotuitong.pngTCM views lower back and leg pain through the lens of both deficiency and excess. The Kidneys are considered the “foundation of the lower back,” so underlying Kidney Qi or Kidney Yang deficiency is often the root cause of chronic, aching lower back pain that worsens with fatigue or cold weather. Superimposed excess patterns include Qi and Blood stasis from injury or strain, and Cold-Dampness obstruction that settles in the lower body meridians, causing stiffness, heaviness, and pain that travels down the leg. The Bladder and Gallbladder meridians are the primary channels affected, as they run down the length of the back and leg.
At TCM Oxford, our treatment for lower back and leg pain combines acupuncture and Tuina to both relieve acute pain and strengthen the lower back to prevent recurrence. Acupuncture is applied to points such as BL23 (Shenshu) on the lower back to tonify Kidney Qi and support spinal health, BL40 (Weizhong) at the back of the knee to unblock Bladder Meridian flow down the leg, and GB30 (Huantiao) at the hip to ease sciatic nerve pain. In addition to reducing pain and inflammation, acupuncture helps regulate pain signaling pathways in the nervous system, providing longer-lasting relief than symptomatic painkillers alone.
Tuina massage plays a central role in relieving lower back and leg pain by addressing both muscular and meridian imbalances. Practitioners use deep rolling, kneading, and pressing techniques on the lower back, gluteal muscles, hamstrings, and calf muscles to release tight myofascial tissue, improve circulation, and reduce pressure on the sciatic nerve. Gentle mobilization of the lumbar spine and hip joints also improves range of motion and reduces stiffness. For patients with deficiency-type chronic back pain, we use warmer, nourishing Tuina techniques to support Kidney energy and strengthen the lower back over time.
For acute lower back strain, 2 to 4 sessions are often sufficient to resolve pain and restore mobility. For chronic, long-standing back and leg pain, a course of 6 to 10 weekly treatments typically delivers significant pain reduction, improved walking and bending ability, and reduced reliance on pain medication. We also provide guidance on gentle core strengthening exercises and ergonomic adjustments to support long-term spinal health and prevent future episodes of pain.
